Two words help explain this: hallucinations and bias. They sound technical, but the beginner idea is simple: AI can invent things, and AI can reflect patterns that are incomplete or unfair.
Learning this does not mean you should stop using AI. It means you should use it with better eyes. The best AI users know how to enjoy the speed while checking the parts that matter.

What is a hallucination?
An AI hallucination is when the system produces information that sounds real but is not reliable or true. It may invent a quote, a source, a policy, a date, a feature, or a detail that was never provided.
Why does it happen?
AI is often predicting a useful response, not checking a live truth database. If it does not know something, it may still try to complete the pattern. That is why confident language is not the same as proof.
What is bias?
Bias happens when an answer reflects unfair, incomplete, or unbalanced patterns. AI can absorb patterns from data, prompts, or assumptions. That is especially important when the answer is about people, hiring, money, health, culture, or opportunity.
How to reduce the risk
Ask AI what it is assuming. Ask for uncertainty. Ask for sources when facts matter, then check those sources yourself. For sensitive decisions, keep a human review step.
